Los misterios del Sistema Nacional de Salud en España son del alma,/npero la Atención Primaria es el libro en el que se leen. Cuando la componente sanitaria del Estado de Bienestar, y particularmente su Atención Primaria, parece estar enferma, y al igual que ocurre con un/npaciente, tanto o más que un diagnóstico se precisa un pronóstico. Este/nlibro pretende contribuir a ambos. La población, los profesionales, la/nindustria suministradora necesita tener un cuadro de cómo se mantendrán unos servicios sanitarios resolutivos financiados públicamente. A/ntodos los actores implicados va dirigido el libro./nLa obra se enfrenta a dos problemas clave: la huída de los médicos de/nla Medicina de familia y el que la carga de enfermedad asociada al uso/ny abuso de medicamentos quede tan solo por detrás de las enfermedades cardiovasculares y del cáncer. Se realizan propuestas tan/ndetalladas como viables para tratar de solucionar ambos problemas./nEl análisis de casos, como la insuficiencia cardíaca y el aborto voluntario, ayuda a entender cómo conseguir que un sistema de salud/ncumpla con sus objetivos de prolongar la vida, evitar el sufrimiento y/nayudar a morir con dignidad./nLos 7 capítulos identifican vías de avance de fácil y necesaria implementación para la muy conveniente refundación de la Atención Primaria.

Asset bubbles and sudden stops in a small open economy

We live in a new world economy characterized by financial globalization and historically low interest rates. This paper presents a simple analytical framework that helps us understand how this new world economy works from the perspective of an emerging economy. Financial globalization gives rise to episodes of large capital inflows followed by sudden stops. Low international interest rates give rise to asset bubbles that pop and burst. The analysis provides novel answers to old questions: What are the effects of asset bubbles on capital flows and macroeconomic performance? How do these effects vary in normal times and during sudden stops? How should policymakers manage capital flows in this new environment?

The chapter analyzes the basis of the market economy in classical Rome, from the perspective of personal vs impersonal exchange and focusing on the role of the state in providing market-enabling institutions. It starts by reviewing the central conflict in all exchanges between those holding and those acquiring property rights, and how solving it requires reducing information asymmetry without endangering the security of property. Relying on a model of the social choice of institutions, the chapter identifies the demand and supply factors driving the institutional choices made by the Romans, and examines the economic circumstances that influenced these factors in the classical period of Roman law. Comparing the predictions of the model with the main solutions used by Roman law in the areas of property, business exchange, and the enforcement of personal obligations allows the chapter to propose alternative interpretations for some salient institutions that have been subject to controversy in the literature, and to conclude with an overall positive assessment of the market-enabling role of the Roman state.

We review a recent literature on cultural differences across euro member states. We point out that this literature fails to address cultural differences between Protestants and Catholics, which are likely a major underlying reason for cross-country differences. We argue that confessional culture explains why Catholic countries tend to have weaker institutions but are more open to economic and political integration. EU policies after the economic crisis looked clumsy and failed to address all concerns, but were viable, caused only a manageable amount of serious backlash and tied in well with Europe’s cultural diversity, also providing scope for learning and adaption.

The organization of public registries: a comparative analysis

This work analyzes the main tradeoffs in the organization of public registries, comparing five forms of organization: (1) the bureaucracies or “discretionary expense centers” used to provide privately valuable services in the welfare state; (2) the internal markets introduced to reform them, and the hybrid solutions that have been used by the liberal state since the 19th century to provide such privately valuable services, including (3) revenue centers based on user fees; (4) franchised systems in which professional civil servants are paid from the profit of an office; and (5) the contemporary variant in which the Government contracts out the provision of the whole service to a private firm. This comparison suggests that hybrid forms allow market forces to play a more effective role in organizing public registries because they are limited to a few variables, which makes stronger incentives possible while, at the same time, reducing the need for extensive planning and supervisory staff.

The achievements of the Spanish economy in the last forty years of the old millennium were impressive, especially taking into account that Spain was largely an agricultural country at the beginning of the twentieth century. Spanish corporations were important protagonists in this catch-up process. This chapter deals with the business development and performance of Spanish big companies in terms of both return on equity and holding return throughout the century. After describing the transformations in the activity of firms by industry, and their corporate ownership, the chapter focuses on these corporations’ results. According to the main outcomes of the investigation, the experience in terms of Spanish business performance in the twentieth century seems to have been one of high profitability. This could be viewed in the light of the fact that the perceived risk was above average, and consequently the returns were expected to be higher than normal.

This chapter focuses on the development of policy analysis tools, mostly policy evaluation and assessment. Spain has a fairly long tradition – albeit not a continuous one – of policy evaluation, starting in the late 18th century, but it has undergone important institutional shifts and setbacks, which reflect major political conflicts. Up until the mid-19th century the major contributions to designing and implementing policies came from outstanding individuals. This was followed by an era of parliamentary commissions that provided a political space to advocate for conflicting interests, a model that later became institutionalised. During World War I a new formula for evaluating and designing policies emerged: the research bureau. Under the Franco dictatorship, the development of policy evaluation suffered a major setback. There was no interest in promoting institutions to assess public policy. Given the very nature of the Franco regime, these institutions only emerged when they were promoted by business leaders and lobbies. As relations with Western countries improved and technocrats became more important in Franco’s government, more policy evaluation was developed and a new university-trained elite took care of it. They became very influential after Franco’s death.
